Whether you're seeking support for yourself or a loved one, we’ll answer your questions, walk you through the process, and help you find the right level of care. Recovery starts with a conversation. We are requesting that all referrals for our residential programs include the following:

- Signed Release of Information

- Biopsychosocial Assessment / Comprehensive Assessment

- Complete Blood Count

- Insurance Information

- Public Assistance or SSD / SSI Information

- Medical History

- Medication List

- Microscopic Urinalysis

- Physical Exam

- PPD (Tuberculosis Test)

- Psychiatric History (if applicable)

- Birth Certificate

- Photo ID

- Social Security Card

For Children entering the Women and Children’s program, include the following children’s documents:

- Birth Certificate

- Immunization Records

- Physical

- Social Security Card

 


INSURANCE:

To make accessing care as simple as possible, our programs proudly accept the following Medicaid plans:

- CDPHP Medicaid

- Fidelis Medicaid

- MVP Medicaid

- Straight Medicaid

Commercial plans will be reviewed on a case-by-case basis.


Once we receive your referral or intake information, our admissions team reviews it within 24–72 hours. We know how important timely care is, and we work hard to get you the answers you need as quickly as possible.
